Virtual Care for Arizona Residents

Mission Wellness & Aesthetics is excited to offer virtual care services! Get the care you need without visiting the office. Virtual care services are available for Arizona residents of all ages! Treatments available for virtual care services include:

General Health: 

-Sinus infections

-Ear infections

-Strep throat

-Acute Bronchitis

-Dental infections

-Nausea or Motion Sickness

Women's Health:

-Urinary tract infections

-Yeast infections / BV

-Hormonal contraception

The cost for virtual care services is $39.99 per consultation. Insurance is not accepted for virtual care services, however if you do have health insurance, it can be used when picking up your prescription(s) from the pharmacy. 

Virtual care treatment forms are reviewed daily between the hours of 9:00 AM and 7:00 PM. Please allow for up to 4 hours during these times for your form to be reviewed. You will be notified when your form has been reviewed and your prescription is sent to your desired pharmacy, or if more information is needed.

Simply fill out the form on this page and attach photos of your credit or debit card for payment as well as your ID. If you are a parent filling out this form for a child, attach your photo ID. 

Virtual Medical Care Disclaimer

The virtual medical care services provided by Mission Wellness & Aesthetics are intended to offer convenient access to evaluation and treatment for certain non-emergency medical conditions. These services are not a substitute for ongoing, comprehensive care provided by a primary care physician or specialist. Patients must agree to pay the cost for consultation at time of service. Fees paid to Mission Wellness & Aesthetics for virtual care services are non-refundable once services have been rendered. Patients are advised that virtual care may have limitations, including but not limited to, the inability to perform a physical examination or order in-person diagnostic tests. If symptoms persist, worsen, or fail to improve with treatment, it is the patient’s responsibility to seek timely follow-up care with their primary care provider or an appropriate specialist. By using these services, patients acknowledge and agree that virtual care is not intended to replace long-term medical management, preventive care, or emergency services, and that any treatment provided should be followed by appropriate in-person medical evaluation when necessary.
